Description Supportive housing and community outreach (attendant care) provided to members of the community with a physical disability 
 
Single storey fully accessible home 
* 24-hour attendant care provided to 12 residents in a co-operative setting 
* maintains a housing waitlist 
* also provides an Outreach Program for approximately 45 to 50 physically disabled adults who live in their own home within the City of Brampton.  
The Agency's Personal Support Workers (PSWs) assist the "consumer" with daily living needs such as bathing, meal preparation, grocery shopping, etc., with some light housekeeping as required.
Hours Residential Service: Daily 24 hrs 
Outreach Service (Attendant Care): Daily 6:00am-12:00midnight

Eligibility Ages: 16 year(s) and up 
 
Applicant : 
-is insured under Health Insurance Act of Ontario 
-has a permanent physical disability 
-is able to clearly direct their own personal support and homemaking services 
 
Will consider out of town applicants
