| Description | Trillium Health Partners is the largest provider of Mental Health Services in West Toronto and Peel region. Our services include both inpatient and community-based services, so care is continued beyond our hospital walls. Our multidisciplinary team offers a broad range of assessment, treatment, education and consultation services to individuals and families from infancy to adulthood. Physician referral for patients to Mental Health Services at Trillium Health Partners can be made through completing the One-Link referral form. One-Link is a single point of access for referrals to 10 Addiction and Mental Health Service Providers funded by the Mississauga Halton Local Health Integration Network.
