Description	Heart House Hospice provides emotional, physical and spiritual support to people, and their families, living with a life-threatening illness * offers case coordination (such as in-home assessment, advocacy, referrals to other services, bereavement information) * home hospice (such as visiting volunteer, relief for caregiver, practical support, local transportation, friendly visiting) * day hospice (such as client programs, caregiver support, family support, respite) * bereavement & spiritual care program * transportation services to the day program are provided to clients of Heart House Hospice only
Hours	Office Hours: Mon-Fri 8am-4pm
Areas Served	Brampton ; Mississauga ; Peel Region
Boundaries	Lakeshore Road north to Mayfield Road; Hwy 50 to Halton Region boundary
Eligibility	Diagnosis: Life threatening or terminal illness with an expected prognosis of less than 12 months.
Application	Self-referral; anybody can refer clients to Heart House Hospice
Fees	None
Languages	English ; Various languages can be provided; depends on volunteer availability
