Description & Services

Description Provides: 
•education to enhance knowledge and skills of staff  
•consultation about learning need  
•care plans and programs related to challenging behaviours of clients / residents  
•assistance with referrals to services and resources and enhanced development of linkages between services providing dementia care 
 
Consultant to: 
•clinical and administrative staff in long-term care facilities 
•community service agencies and Home and Community Care Support Services Central West 
•Adult Day Programs 
•Homemaking agencies 
•Supportive Housing 
•Respite Care Programs dealing with dementia and mental health issues

Hours Mon-Fri 8:30am-4:30pm
Eligibility Referrals can be made by anyone; within a government funded community agency; who has identified a learning need related to the mental health of the elderly; or anyone concerned about an older adult who may have a psychological, behavioural or memory problem
Application Must be a government funded agency such as: Home and Community Support Services; Adult Day Programs; Homemaking agencies; Supportive Housing; Respite Care programs dealing with dementia and mental health issues
