DescriptionThis is a consultative palliative care program that supports people with life-threatening illnesses to remain in their home. APNs have specialized training in palliative care and an extensive knowledge of the palliative system to provide client care and family support throughout the continuum of care and disease trajectory. APNs work closely with physcians, nurses, and other community providers to support them in providing care to their clients/patients and their families.
HoursCurrently Monday- Friday 8:30 am-5:00 pm 
When the program is fully implemented, a 24 hour/day, 365 days a year approach will be available.
