Description | A partnership between Services and Housing In the Province (SHIP), Punjabi Community Health Services, and Peel Senior Link * offers supportive and health services that maximize the ability of high risk seniors to live independently in their homes Makes in-home visits within North West Brampton The team is comprised of counsellors, mental health counsellors, case managers and personal support workers. Services include: • 24-hour on call system for emergency crisis situation • risk management assessment and education • personal support services • access to supportive housing options • chronic disease management through linkages with existing community-based health programs • falls prevention training, assessment and management of environmental and home risk factors SHIP Mission: To optimize quality of life for individuals with mental illness through housing and community-based services. SHIP provides a variety of community based mental health and supportive housing services for men and women who have an identified psychiatric illness, enabling them to live independently in the community in an atmosphere of support, co-operation, understanding and affordability |
