| Description | Provide non-medical home care services 24/7 to all ages at Home, Retirement home, Long term care or Hospice or Hospital, on hourly basis or 24 hours round the clock, live-in care. Services includes bathing, grooming and hygiene, transfers, meal preparation, medication reminders, safety supervision, transportation needs for personal and social appointments, chore services, light housekeeping, Foot care and family respite. Specialize in palliative care, clients with Alzheimer's disease and Dementia. Also provide assistance for new moms. Highlights of care: Plan of Care developed by Registered Nurse based on RN Assessment Care oversight by RN Continuity of care Guaranteed. Quality of care assured. Transition of Care from Hospital to Home and care co-ordination. Virtual Care Services: Provide Remote Care using Video phone, medication reminders, enhance social connectivity and care co-ordination with other caregivers. Family Portal: Daily, weekly updates on the wellness of the client, ESAS and PPS updates. We support independence and dignity of Seniors | |||
