Description	Bramalea CHC is committed to working with the older adult population, 65 or older.    Seniors registered as clients at Bramalea CHC can access primary health care from a team that includes a nurse practitioner, doctor, registered nurse, dietitian, physiotherapist, social worker and community health educator. The goal is to break down barriers seniors face accessing the care they need.     Services Provided:  Home visiting * Medication management * Counseling and therapy related to mental health, healthy aging, abuse and other related health matters    Recognizing that isolation is a problem many seniors face, Bramalea CHC works at reducing this isolation so they can make the most of the great experiences and skills they have to offer. Some plans to engage seniors include:  Monthly brunches * Media program * Arts workshops

Eligibility	Ages: 65 year(s) and up    - seniors (65+)  - newcomers to Canada (less than 5 years)  - racialized communities  - people facing menal health conditions, abuse or substance use issues
