|Description||A service for older adults and seniors with health challenges like depression, anxiety, or other mental health concerns. |
Who can use this service:
Seniors (65 yrs and up) with a mental health issue (dementia excluded).
Adults over 55 yrs can also be seen if they have complex medical issues in addition to the mental health issue.
How this service works:
After the intake referral is received, a case manager will follow up with you to complete the intake process and assess your needs. Based on the assessment, the case manager will provide short term or long term case management. This includes building a supportive partnership, providing assistance in managing illnesses, and linking them with other community resources.
How to request this service:
Anyone can make a referral for Treat at Home (an individual, a family member, or a professional) Referrals can be made by telephone , or sent electronically by downloading the referral form from CANES website.
