| Description | A service for older adults and seniors with health challenges like depression, anxiety, or other mental health concerns. Who can use this service: Seniors (65 yrs and up) with a mental health issue (dementia excluded). Adults over 55 yrs can also be seen if they have complex medical issues in addition to the mental health issue. How this service works: Seniors receive an intake assessment in their home (or other location convenient to the client) and short-term case management support from a psychogeriatric nurse. If more intensive support is required, a case manager will work with clients to build a supportive partnership, provide assistance in managing illnesses, and link them with other community resources. How to request this service: Anyone can make a referral for Treat at Home (an individual, a family member, or a professional) Referrals can be made by telephone , or sent electronically by downloading the referral form from CANES website. | |||
