DescriptionAssertive Community Treatment Team (ACTT) is an intensive community- based mental health service provided by a multi-disciplinary team for people with serious mental illness, functional impairments and complex needs.  
The ACTT team goals are based on the individual needs of each client and focus on reducing hospitalizations, improving quality of life, support to families and caregivers, increasing housing stability and reducing symptoms of mental illness. The ACTT team provides psychiatric, rehabilitative and support services including case management, crisis assessment and intervention, individual supportive therapy, vocational assessment and job development, and other assistance.

EligibilityAges: 16 year(s) and up 
 
Adults 16 years and older who have serious mental illness, functional impairments and complex needs. The ACTT team determines eligibility based on the information on the ACTT referral form.
ApplicationA completed ACTT referral form is sent to the team and can be sent by fax, mail, or in person. Professionals and other providers can call to discuss eligibility. Completed Form 14 is required for the ACTT team to access information from current/previous service providers, hospitals and other significant supports.
