| Description | Day programs, uniquely designed for individuals with Alzheimers disease or related dementia, are offered in a secure environment. Appropriate for individuals diagnosed with memory loss/dementia who are not able to participate in other social programs. Adult Day Services activities include: Chiropody service – 1x/month * exercise * Music therapy * Hair dressing-1x/month Program also has ADL assistance (toileting, incontinence care, feeding, one or two person transfers) Transportation available. | |||

| Application | Referral can be made from anyone. Eligibility is determined by the CCAC ; There is an specific day program assessment required once space is available which takes 1 - 1 1/2hrs to complete and focuses on activities of daily living and social history | |||
