Description/Eligibility/Service Details
DescriptionIn partnership with two other community agencies, Trillium Health Centre provides housing support through case management in Etobicoke and York. Units are leased by Supportive Housing in Peel (SHIP) for clients who have severe and persistent mental illness, and are either homeless or at risk of homelessness or are using the hostel system. Trillium case managers work in partnership with clients to develop the skills and supports they need to live successfully in both their housing unit, and in the community.
