| Description | The mental Health Adult Day Treatment Program (ADTP) provides services to adults 18 years of age and older who have a diagnosis of a mental illness. The ADTP promotes a recovery model through group topics that are covered are assertiveness, stress and anxiety management, life skills, self-esteem, relaxation, healthy lifestyles, creative expressions, leisure education and exercise. The program also teaches medication management, symptom management skills and information on other mental health related topics. There are two phases to the program. Phase One is the Active Care Phase that lasts 12 weeks where participants are expected to attend 4 days a week. Phase Two is the After Care Phase that lasts another 12 weeks. Throughout these 6 months, participants are assigned a case manager who will provide supportive counseling, help identify goals for treatment and assist the participant to attain these goals. The case manager will also monitor the participant’s progress, provide support and guidance and assist with the transition from the ADTP to community reintegration. While participants are in the Active Phase of the program they will see our program psychiatrist approximately once every two weeks. During those 12 weeks, we ask the each participant stop seeing their own psychiatrist for that period of time. The can resume their usual follow-up appointments with their regular psychiatrist after those 12 weeks. If participants have a medical concern, they are encouraged to see their family doctor. | |||
