DescriptionAccess to Recovery provides intensive community support for individuals with long term mental illness. Clients develop skills and resources that assist in their recovery process. Program is run by a team of mental health professionals with training in case management and mental illness. 
 
Clients can expect: 
- Supportive partnership with a community support worker 
- Assistance in understanding mental illness and the recovery process 
- Guidance in developing a recovery plan and reaching goals 
- Linkage and coordination of community resources 
- Liaison and coordination of community resources 
- Liaison with family, friends and other service providers 
- Meetings at home or in the community 
 
HoursMon-Fri 9 am-5 pm (some flexibility to meet the needs of individuals)

EligibilityAges: 16 year(s) and up 
 
Living, or intending to live, in Brampton, Malton and Caledon. 
Living with severe and persistent mental illness, including: concurrent disorder and dual diagnosis
