| Description | Reaches out to homeless individuals by mobile van "reconnaissance" trips (responding to tips and referrals from churches, Street Help Line, agencies and general public, by visiting shelters and other areas frequented by homeless people). Outreach identifies and informally assesses potential clients who may need support services and initiates referrals to appropriate services. Assesses assess potential clients' housing and related needs; provide information about options available; confirms their interest in the program; refers elsewhere as needed; and follows waiting list procedures as appropriate. INFORMATION COORDINATION/DISSEMINATION Project team becomes familiar with, and is current with, relevant legislation, information on application procedures and waiting lists for specific housing support services and financial assistance. Outreach team identifies and matches specific client needs with and filling units as available. Assists clients when viewing/applying for housing. Staff are trained in landlord and tenant relations, develops a knowledge base and imparts this information to clients. ASSERTIVE REFERRALS Initiates referrals for services needed by client and persists assertively until services are provided. When it is perceived that an individual is being discriminated against or does not possess the assertiveness to challenge their entitlements, provides client with information and encourage confidence to advocate for him/herself or advocates on his/her behalf when necessary. COMMUNITY SUPPORT PARTNERSHIPS Program representatives participate in committees, coalitions, and forums to ensure that systemic advocacy is pursued on behalf of homeless individuals. | |||
