Description	Assertive Community Treatment (ACT) is a multidisciplinary team of mental health professionals comprised of a psychiatrist, psychiatric nurses, social workers, occupational therapist, substance abuse specialist, a vocational counsellor and a peer support worker.     The ACT team provides client-centered recovery-oriented treatment for individuals who live with a serious mental illness.    This multidisciplinary assertive outreach team also addresses the needs of individuals with a concurrent disorder or those who have come in conflict with the law or have the potential to do so.
