Alternate Name	ACTT; ACTT Team
Description	A community program for people with severe and persistent mental illness, who require ongoing, comprehensive and individualized mental health services. This service is intended for people who have has a minimum of 60 hospital days in the previous 2 years.  The team works in partnership with their clients to decrease the need for hospitalization and improve the clients quality of life. Assertive outreach is provided to clients who have had difficulty maintaining contact with mental health services. This multidisciplinary team, which includes a psychiatrist, provides treatment, rehabilitation and support services in the client's home or in the community.
Hours	*Mon-Thurs 8:30 am-8:30 pm  *Fri 8:30am-4:30 pm  *Sat and holidays 8:30-4:30  *24/7 On-call crisis response service to current ACTT clients
