Description/Eligibility/Service Details
DescriptionComprehensive psychiatric assessment for ambulatory seniors clients at high risk of requiring admission to hospital for psychiatric treatment. 
Comprehensive assessment and follow up home visits by a geriatric psychiatrist and /or member of the clinical team may be part of the assessment and ongoing treatment plan. 
This service is available to clients living in their own homes, retirement homes and long term care facilities.
