| Description | Mary Centre provides residential services to adults who have developmental challenges in Brampton, Toronto & Woodbridge Brampton Residence * two wheelchair accessible homes for adults (four people in each house)* residents participate in education, employment, and recreation activities in the community Supported Independent Living Program * apartments in Brampton, Toronto, and Woodbridge where residents live independently with some staff assistance Community Support Day Program * provides support to individuals with a developmental disability; allowing for opportunities to interact with and be active with communinty setting* develop life and social skills in group setting* Integrated Seniors Program * supports people (50yrs and older) to choose leisure, education, and/or volunteer opportunities that encourage integration with with their peers * Peel and Toronto Respite Program * provides relief to caregivers of adults with a developmental disability * in-house support workers provide assistance in areas such as life skill development, social integration, and access to recreation activities * out of home option - St Bernard's residence, Toronto * 5 respite beds (not wheelchair accessible) * fee for service Community Support Program - Peel * assists individuals to access the supports required to develop the skills for daily living and achieve a more independent lifestyle while living in the family home * matches Support Workers to the individual * provides opportunities to meet new people, develop new friendships, and develop life and social skills in community settings. Transtion and Long term Care Program * assist individuals to prepare for life in long term care while living in their current situation* support and maintaining a connection with the individual once the long term care placement had occurred * assist individual to create social networks within the long term care community. Volunteer Program * enhances the lives of the individuals with "Friendly Visitors", "Drivers to appointments & Church", "Literacy/Computer Tutors". Faith Outreach Program* connects individuals with the faith community of their choice, and helps them become more involved with groups and activities in their faith communities. | |||
