Description/Eligibility/Service Details
DescriptionThe Adult Protective Service Worker works with adults 18 and older who have a developmental disability and are living on their own in the community. The worker will assist the individual to identify their strengths and needs and provide support, problem-solving, and life skills. Coordination and case management are also provided along with advocacy and guidance to the appropriate community resources.

EligibilityAges: 18 year(s) and up 
 
* A resident of the Region of Peel. 
* A person who has a developmental disability. 
* Adult aged 18 years or older. 
* Living on your own or planning to do so in the near future. 
* Have no significant social supports. 
* Prepared to work actively with an APSW to form a support plan. 
* Self-referred to program.
