Description	The renal dialysis facility has a capacity of 80 haemodialysis stations (combining 56 treatment stations in two areas of the hospital with the additional 24 stations located off site at Watline Ave) * services include: Home Peritoneal Dialysis Unit - provides a multi-disciplinary program of care, education and support, in an outpatient setting, for patients with end stage renal disease who wish to maintain their independence by performing their own peritoneal dialysis at home * each patient is assigned a primary nurse as they come onto the program who assesses the patient and plans their care along with the patient, family and other team members * each patient completes a customized education program before performing their own home peritoneal dialysis Predialysis Clinic - provides a multi-disciplinary approach to optimizing wellness in patients approaching end stage renal disease in order to delay the imitation of dialysis for as long as possible * provides ongoing education to better manage hypertension, anemia and diabetes * each patient in this clinic is assigned a primary nurse who directs their care along with the support of the dietitian, pharmacist, physician and social worker Transplant Clinic - provides follow-up care for renal transplant patients who have undergone a kidney transplant at a transplant centre and whose condition is now stable enough for follow up in their own community * each transplant patient is assigned a primary nurse who directs their care along with the support of the multi-disciplinary team as necessary
